Blue Care is after a Home Care Package Partner to join our team at Toowoomba Community Care. This role will partner with clients and their families/carers to ensure that their needs are met in accordance with their goals and desired outcomes, consistent with Blue Care’s Tailor Made service model, culture and values and will facilitate choice in the type of services and option available.
You’ll need your Cert III in community/Aged Care or equivalent, demonstrated experience in coordinating or assisting in the care coordination of a community-based service or client care, knowledge of Consumer Directed Care facilitation and a current driver’s license. Experience of budget processes at the service and client level and case management/coordination experience would be advantageous. Your excellent interpersonal skills and understanding of person centred care will also make you a great candidate. You will also be proficient with using tablet computers and smart phones. Prior experience in Community Care and or Home Care Package Coordination is highly advantageous.
