Position Summary

  • $95,000 + 10k Vehicle Allowance + Super + NFP Salary Packaging

We're seeking a Care Coordinator to support exceptional care delivery for our community clients across Bundaberg, Hervey Bay and Maryborough!


Working with your Care Manager, you’ll utilise your skills and experience to drive outstanding care for our community clients! With brand new leadership team and service delivery structure, there's never been a better time to join Blue Care.


You’ll have the opportunity to lead and inspire your team providing supervision, guidance and mentorship, through your effective decision making and clinical experience. You’ll provide exceptional care whilst responding to clinical nursing issues and concerns. With their wellbeing always in mind, you will review resident care and care plans promoting independence & quality of life, drive community funding and make a real difference to the quality of care we deliver. This includes:

  • Development, implementation, monitoring and review of care plans
  • Comprehensive and accurate assessment of community clients
  • Assisting with the orientation of new staff and ongoing professional development
  • Support in ensuring the service meets all accreditation standards         

What you’ll need

You’ll be an experienced clinician, with considerable experience within an acute, community or aged care environment. You’ll be an advocate for person-centred care, leading by example through exceptional communication and customer service. You’ll present with:

  • Bachelor of Nursing, or equivalent qualification 
  • Current AHPRA registration
  • Experience coordinating care provision within a large team
  • Confidence to manage conflict, pressures and competing priorities
  • Ability to provide leadership, support and mentorship to other team members

Supporting our clients in the best way possible is important to us. To be employed by Blue Care, you will be required to complete the NDIS Worker Module prior to commencement and provide a copy of your certificate of completion.

Selection Criteria Desirable: Do you have experience working within and leading a multidisciplinary team?
Desirable: Do you have extensive leadership and management experience and knowledge in residential and or community care services and in area of speciality?
