We’re inviting applications from experienced Lead Practitioners to join our Coomera Community Care Team!


The Role:


In this role you will act as the lead disability practitioner at a service level providing guidance and leadership to employees delivering disability services.  In addition you will work closely with individuals and their families throughout the NDIS planning process to agree on support plans and convert those plans into service agreements and service bookings to allow individualised billing consistent with NDIS pricing schedules.


You will have excellent leadership capabilities with the ability to manage multiple priorities and stakeholders, along with highly developed interpersonal communication skills to assist you in building rapport with the person we support, their family and their support teams.  In addition you will have experience with service delivery and redesign at an individual and group level preferably within disability


What you’ll need:

  • At least 12 months’ experience supporting service delivery and service redesign at an individual and group level, preferably with people with disability
  • Experience in or knowledge of the National Disability Insurance Scheme
  • Excellent communication skills
  • Intermediate - advanced computer skills 
  • Post school qualifications in a related field (or equivalent professional experience)
  • Blue Card and Yellow Card or willingness to obtain
  • Current Australian drivers licence and roadworthy vehicle as required
